These forms are specified by the Administrative Regulations as part of the rules. These forms were last updated in 2023 – ensure you have the latest version should you require these forms.

Form B – Union Affiliation Audit Certificate (sample here) – please contact the General Returning Officer by email if you require this form (returning.officer@qld.alp.org.au).

Form E – Nomination for Candidate for Preselection for Public Office (sample here) – please contact the General Returning Officer by email if you require this form (returning.officer@qld.alp.org.au). A form for federal candidates is also available.

Form F – Nomination for Candidate for Internal Plebiscite (sample here) – please contact the General Returning Officer by email if you require this form (returning.officer@qld.alp.org.au). Separate nomination forms for Equity Groups are also available.

Form G – Authority to Deduct Parliamentary Levy (sample here) – please contact the General Returning Officer by email if you require this form (returning.officer@qld.alp.org.au).

Form K – Residency Verification – for those who are unable to enrol (under 18s and permanent residents) but would like to vote in ALP plebiscites (not preselections) – see Rule M3.2.

Form L – Application for Register of Postal Voters – for those who cannot vote online and wish to receive a postal vote.
